ARIEHome Care

After-hospital care

The first month home is where recoveries succeed or unravel. We make sure it succeeds.

Nearly one in five older adults discharged from hospital is readmitted within 30 days, most often because of a fall, a missed medication, dehydration or a missed follow-up. Every one of those is preventable with a trained person in the home. That is what this service is.

Built for the discharge plan

We can meet you at the hospital or rehab before discharge, read the plan with the care team, and have a caregiver waiting at home the day you arrive. Our nurse translates the discharge instructions into a simple daily routine the whole family can follow.

What we do in the first 30 days

  • Safe pickup and settling in at home
  • Medication reminders and pharmacy runs
  • Help with walking, stairs and transfers
  • Bathing with wound and incision awareness
  • Meals that match the discharge diet
  • Rides to every follow-up appointment
  • Daily notes on pain, appetite, swelling, mood
  • Prompt escalation to the family or doctor

Common situations

Hip and knee replacement. Heart failure and COPD flare-ups. Stroke recovery alongside home therapy. Pneumonia. Falls and fractures. Cancer treatment cycles. Any surgery after which the surgeon said “do not be alone for the first week.”

Works alongside home health

If Medicare has ordered skilled home health (a visiting nurse or therapist), we fill the hours in between. Those visits are short; recovery happens in the long stretches between them, and that is where our caregivers are.
